HealthSnap’s Hypertension Study: Results

In 2023, HealthSnap partnered with Prisma Health and Virginia Cardiovascular Specialists for a pivotal study titled "Asynchronous Management of Hypertension: The Effectiveness of Remote Patient Monitoring in Mixed Populations." This study evaluated RPM’s impact on hypertension management across diverse patient groups, setting the stage for the remarkable outcomes we continue to achieve today.

The study involved 2,761 patients who committed to taking 30 or more blood pressure readings over at least 90 days. Using cellular-enabled blood pressure cuffs, patients transmitted their data to HealthSnap’s EHR-integrated Virtual Care Management platform. Our licensed clinical staff, now numbering over 400 Licensed Nurse Practitioners (LNPs) as of 2024, closely monitored the readings, analyzed health trends, and collaborated with patients to ensure adherence to evidence-based care plans. This collaborative approach underscored the power of technology and human connection in driving meaningful change.

Key Findings from the Study

The results of this study highlight RPM’s potential as a vital tool in hypertension management:

  • Overall Blood Pressure Improvements: Across all patients, we achieved an average systolic blood pressure reduction of 10 mmHg (p < 0.0001), with corresponding improvements in diastolic pressure.
  • Cardiology Patients: Systolic BP decreased by 11.4 mmHg, and diastolic by 5.6 mmHg.
  • Primary Care Patients: Systolic BP dropped by 8.0 mmHg, and diastolic by 4.6 mmHg.
  • Stage 2 Hypertension: Patients with more severe hypertension saw even greater improvements. Cardiology patients experienced a 20.8 mmHg systolic reduction and 13.9 mmHg diastolic reduction, while primary care patients saw a 15.9 mmHg systolic drop and 10.0 mmHg diastolic drop.
  • Pulse Pressure: Patients with high pulse pressure benefited as well, with reductions of 6.2 mmHg (primary care) and 5.8 mmHg (cardiology).

We’re proud to report that 68% of our patients with high blood pressure now have it under control—a testament to the effectiveness of our approach. This achievement means more patients are living healthier lives, with a reduced risk of heart attacks, strokes, and other complications, giving them greater peace of mind. Our clinical results are equally impressive. Patients enrolled in our programs have achieved an average 14.4 mmHg reduction in systolic blood pressure. This reduction is significant because it can lower the risk of serious cardiovascular events by up to 30%, according to a 2021 Lancet meta-analysis (Rahimi et al.). In practical terms, this means fewer health emergencies for patients and a lighter burden on healthcare systems—a win for everyone involved. For patients with Stage 2 hypertension, the outcomes are even more striking, with some achieving systolic reductions as high as 23.4 mmHg.

The 2025 report also highlights the power of dual enrollment in RPM and CCM. In a comparison of 453 dual-enrolled patients versus 531 RPM-only patients with Stage 2 hypertension, those in the combined program saw a 16.72 mmHg systolic BP reduction—55% greater than the 10.82 mmHg reduction in the RPM-only group. Dual-enrolled patients also achieved a 36.2% blood pressure control rate, nearly double the 19.96% rate for RPM-only patients, alongside higher engagement rates (64.9% vs. 49.3%). These findings underscore the synergy of combining continuous monitoring with personalized care management.
